If you have any questions or concerns about getting authorizations for medical services, please contact Member Services at 1-866-783-1444, (TTY for the hearing impaired: 711), and a representative will be able to assist you.
 
To find out whether you are enrolled or eligible for Medicare or Medicaid, contact Member Services at 1-866-783-1444 (TTY for the hearing impaired: 711). One of our representatives will be able to assist you.
 
Call Member Services at 1-866-783-1444 (TTY for the hearing impaired: 711) and notify a representative of the change, so that we can update our records and ensure that your services continue properly.
 
To protect your privacy, we need your permission before we allow anyone else access to your private information. There are three ways that you can authorize a family member or friend to serve as your representative with access to information about your care and services with VNSNY CHOICE.
 
1. Write a letter indicating that you, as the member, authorize your representative to speak on your behalf. Include the full name of your representative, a brief description of your relationship, and the sort of information to which you are granting access. Send this letter to Member Services for processing:

2. To give your representative access without limitations, you can ask Member Services to mail a Health Information Release form to your home. You can do so in writing at the above address, or by calling CHOICE Member Services at 1-866-783-1444 (TTY for the hearing impaired: 711).
 
Then, fill out the form and send it back to Member Services for processing.
 
3. Call Member Services at 1-866-783-1444 (TTY for the hearing impaired: 711) with your authorized representative and give verbal authorization. Your authorization will be valid for 14 days from the date of the call.
 
To protect your privacy, Member Services will always ask you to provide three pieces of information to identify yourself. These could include:
  • Your CHOICE Member ID Number
  • First and Last Name
  • Date of Birth
  • Full Address
  • Medicare ID Number
  • New York Medicaid ID Number
  • Social Security Number
 
If you need your services suspended because you will not be at home — for example, if you go away on vacation, or have to go into the hospital — please contact Member Services at 1-866-783-1444 (TTY for the hearing impaired: 711), so that the appropriate arrangements can be made.
 
Yes. If you are dissatisfied with the quality of care that your aide is providing, please contact the agency that provides your aide. If you are dissatisfied with the agency, please call Member Services at 1-866-783-1444 (TTY for the hearing impaired: 711).  
 
Yes. If you will not require HHA services on your usual schedule, please contact Member Services at 1-866-783-1444 and a representative will notify the team coordinating your care of the change.
 
Contact Member Services at 1-866-783-1444 (TTY for the hearing impaired: 711). If you have already chosen a new doctor, give our representative the doctor's name. Once the representative confirms that the doctor participates in your CHOICE plan, a new ID Card will be mailed to you.
 
If you have not chosen a new doctor, you can find one using our Provider Search tool, or call Member Services at 1-866-783-1444 (TTY for the hearing impaired: 711) to help you locate an In-Network Primary Care Physician.
 
If your home address changes, contact Member Services at 1-866-783-1444 (TTY for the hearing impaired: 711). If your new address is within our service area for your plan, your coverage will not be affected. If you move outside of the service area, you may no longer be eligible for our plan, in which case you would be disenrolled. To check whether an address is within your plan's service area, call Member Services or follow this link.
 
If you need help identifying a provider who meets your needs, you can contact CHOICE Member Services at 1-866-783-1444 (TTY for the hearing impaired: 711).
 
Contact Member Services at 1-866-783-1444 (TTY for the hearing impaired: 711). Please have the bill with you when you call, as the representative will ask you for specific information found on the bill. The representative will work with you and your doctor to clarify the bill and determine whether you may be responsible for any charges.

Visiting family or friends outside the U.S.? As a CHOICE Medicare member, you'll have more peace of mind knowing that you have up to $50,000 in worldwide coverage for emergency services and urgent care when you travel outside of the U.S.

Some plans are available to anyone who has both Medical Assistance from the State and Medicare. Premium, co-pays, co-insurance, and deductibles may vary based on the level of Extra Help you receive. VNSNY CHOICE Medicare Total is an HMO SNP plan with a Medicare contract. This plan is also a Medicaid Advantage Plus plan, with a contract with the New York State Department of Health. Enrollment in VNSNY CHOICE Medicare depends on contract renewal.

The formulary, pharmacy network, and/or provider network may change at any time. You will receive notice when necessary.
