VNSNY CHOICE Total (HMO SNP) members have many questions that can be answered by calling our professionals at VNSNY CHOICE Member Services. 
Just explain your question or concern to them, and they’ll often be able to address your need right then and there. The Member Services number is:

If Member Services cannot answer your question, they will identify the right member of your care team to get you the help you’re looking for.
VNSNY CHOICE Medicare Total is an HMO SNP plan with a Medicare contract. This plan is also a Medicaid Advantage Plus plan, with a contract with the New York State Department of Health. Enrollment in VNSNY CHOICE Medicare depends on contract renewal.
